6th Grade Field Trip to Cleveland
Cost $20.00 for students
$12.00 for parent chaperones

(The Field Trip is in the 7th week of the 9 weeks)
Failure to comply satisfactorily with the following pre-activity requirements
will result in denial of participation in the privilege of attending the
activity.
1. Unpaid fees, fines, lunch charge slips, and fundraiser monies.
2. 2 Unsatisfactory Conduct Cards received in the 9 weeks.
or
2 Bus Discipline Referrals received in the 9 weeks.
or
A combination of Unsatisfactory Conduct cards and Bus Referrals totaling
2 received in the 9 weeks.
* Lunch misconduct forms may be considered at the 6th grade team discretion
3. Suspensions/Alternate School
If one out-of-school suspension has been assigned during the 9-weeks.
or
Any alternate school placement that is assigned because of an unsatisfactory
conduct card within the 5-days before the day of the activity.
or If two such alternate school placements have been assigned during the
current 9 weeks.
4. Truancy - if a student is truant for one or more periods within the
5 days before the activity or if truant for one or more periods twice
during the 9 weeks.
5. Behavior Communications - 4 Behavior Communications received in the
9 weeks. (ZAPS will not be included in this )
6. If a student is participating in a Behavior Plan, the student will
be denied privileges when reaching Phase Two ( three infractions in one
day) for the 2nd time.
7. Previous suspensions from other trips during the year.

If the student has been suspended out of school for 5 or more days anytime
during the current semester or 10 days anytime during the year.
or
Any in-school suspension that is assigned because of an unsatisfactory
conduct card within the 5-days before the day of the activity.
or
If two such in-school suspensions have been assigned during the current
9 weeks.
4. Truancy - if a student is truant for one or more periods within the
5 days before the activity or if truant for one or more periods twice
during the 9 weeks.
